The Charge Nurse will be responsible for the day to day management of their assigned neighborhood floor. They will collaborate with the Social Worker to ensure adherence to DPH regulations and will role model and assist with the implementation of culture change initiatives. The team leader will assume the charge role position and provide direct care as needed and directed. The Charge Nurse will mentor and coach staff and assist with monitoring and achievement of quality indicators and Quality of Life metrics.

Support day to day management of the floor in collaboration with the Chief Nursing Officer, Social Worker, and the Nurse Manager.
Provide supervision of all nursing staff. Participate in completion of performance reviews as assigned.
Assist with the monitoring of clinical and quality indicators and collaborate with the Associate Chief Nursing Officer/Nurse Manager to identify system and process improvement plans to improve results.
Assist with the oversight of DPH regulations—coordinating a team effort to be DPH survey ready at all times.
Ensures all care delivered is within the scope and established standards of safe professional nursing practice and HSL core competencies and policies.
Collaborates with other members of the clinical support team (MD, NP, nurses, social workers, therapists, dieticians, life enhancement coaches) to meet the clinical needs of the patient and monitor outcomes of care.
Adheres to best practice recommendations and implements evidence based interventions to ensure optimum outcomes for patients.
Practices in a primary nursing model and maintains accountability for a set of assigned patients/household.
Collaborates and participates in care planning activities.
Ensures adherence to the plan of care, revises as necessary, and ensures the plan accurately reflects an individualized approach to patients’ current needs.
Documents per policy in the electronic medical record.
Identifies and seeks out necessary resources as needed to ensure the provision of safe and effective care.
Delegates and supervises clinical care delivered by the nurses, nursing assistants, and dieticians.
Assumes accountability for his/her own learning needs and professional development. Completes all monthly online education tools.
Demonstrates critical thinking abilities in problem resolution.

Actively participates in shared governance, committees, etc.
Collaborates in the development of new work processes and systems.
Identifies and responds to safety concerns on the household neighborhood.

Participates in performance improvement projects.
Participates and assists with daily routine of the patients (i.e. assist with serving meals)
Serves as a coach and mentor to all staff in promoting quality of life and engagement efforts on the HH level.
Will be assigned charge nurse responsibilities.
Responsibilities may include:
Giving face-to-face warm hand off report.
Provide clinical/supervisory oversight to assigned household floor during shift.
Giving warm hand off report to coordinator.
Provide clinical/supervisory oversight to assigned unit during shift.
Delegate assignments to team assigned to shift/charge nurse.
Initiate necessary reports including DPH, incident reports, concern resolutions, (RL) solutions.
Assure completion of admission or discharges during shift.
Attend rounds (wound, physician, interdisciplinary team, etc.).
Provides oversight of LPN assessments.
If acting in the capacity of charge nurse, may develop patient care assignments (appointing other staff, RN, LPN or PCA to implement an established care plan for a designated period of work time), create schedules, and contribute to performance evaluations and educating other staff on HSL policy and procedure.
Graduate of approved school of nursing required; BSN preferred
