    Under the guidance of the Director of Quality and within established agency policy and procedures, the Assistant Director provides support and assists the Agency in meeting the objectives of the Quality and Safety Department which include but are not limited to meeting regulatory requirements and achieving clinical excellence by supporting OASIS accuracy, supporting documentation accuracy, performing data analytics review and response plan development, participating in staff orientation, identification of individual and team educational needs and development and implementation of a response plan in collaboration with Clinical Leadership, and general performance improvement activities. This role is guided by the principles of the Triple Aim – achievement of clinical excellence met in a financially responsible manner and resulting in strong patient outcomes and high patient satisfaction.

     

    Responsibilities

    • Provides excellence in service to internal customer base of employees and external customers including patients, referral sources and physicians and in accordance with agency standards.
    • Works collaboratively with all team members, Executive and Clinical Leadership and office support staff to coordinate high quality educational activities based on identified need.
    • Participates in implementation, evaluation of the clinical orientation program for all licensed professional disciplines that is customer focused, competency based, and in compliance with regulatory standards and agency policy.
    • Accompanies clinical staff during home visits to observe skill level and accuracy with adherence to regulatory requirements and best practice clinical skills. Reinforce regulatory guidelines, OASIS accuracy, agency policies and procedures and facilitate optimum levels of clinical practice.
    • Assists Clinical Director and Quality Director with developing and providing educational programs for licensed professional staff that will improve individual and group performance on meeting the Triple Aim.
    • Provides individualized education sessions based on review findings from home visits and collaboration with Quality and Patient Safety team members and Clinical Leadership, and as identified during and after completion of orientation.
    • Contributes in developing and implementing the annual education plan, annual competency program, new program protocols, and in attaining relevant goals.
    • Participates in staff meetings to facilitate ongoing education as it relates to clinical, quality, regulatory and compliance standards.
    • Follows agency policy and procedures and adheres to standards of practice within agency guidelines.
    • Administers care within scope of practice, as indicated
    • Participates in the implementation and evaluation of the agency quality program to assure quality outcomes are achieved.
    • Participates as required in audit practice of clinical records for appropriate utilization of services, quality outcome measures, standards of practice, and to ensure regulatory compliance.
    • Under the direction of Quality Director, reviews risk-adjusted outcomes, Value-Based Purchasing data, utilization outcomes and adverse events on quarterly and annual basis.
    • Utilizing Home Health Gold, data analytic software system, reviews outcome data of both agency and individual clinicians to identify trends, areas requiring outcome improvement, and assists in developing, implementation and revises as necessary a response plan
    • Participates in carrying out general functions of the Quality and Safety Department.
    • Participates in DPH Surveys as requested.
    • Participates on the Home Care Quality and Safety Committee and other committees as requested by Quality Director.

     

    • Participates in clinician orientation to electronic medical record and provides ongoing support.

     

    Qualifications

    • Licensed RN in the state of Massachusetts.
    • Baccalaureate degree from accredited program (Equivalent combination of education, training, and experience may be substituted for this criterion).
    • Must have recent home health experience for minimum of three to five years
    • Must have demonstrated experience in OASIS, Medicare Home Health Regulations, interpretation of outcome data, and clinical staff education.
    • OASIS and ICD-10 Coding Certification preferred
    • Experience in Quality/Safety environment.
    • Experience in computer technology, home health systems, Allscripts/Netsmart knowledge a plus.
    • Possess strong leadership skills, effective communication skills, ability to independently work in a team environment.
