    The Intake Coordinator will coordinate all referrals received. Responsibilities include screening of clinical information on referrals to ensure that appropriate orders and discharge information is received. The Intake Coordinator will work with Billing Dept to provide any clinical information required to obtain initial authorizations and will maintain communication regarding tracking of insurance authorizations about to expire. The Intake coordinator will be responsible to obtain ongoing insurance authorizations as needed. The Intake Coordinator will have an active role in our Tele-health Program. Responsibilities also include assisting in answering and directing calls coming into the main home care line and assist in triaging patient and family calls and working closely with the Scheduler regarding scheduling of patient visits.


    Hours: 9:00 - 5:30pm


    • Assist in answering and directing calls that come into the main home care line.
    • Triages patient inquiries and assists Clinical Managers and Scheduling Coordinator in communication and scheduling of staff as needed.
    • Responsible for obtaining referral information from all referral sources- direct referral source phone calls, 4Next and Allscripts referral management system utilizing e-fax.
    • Ongoing communication with Home Care Liaison Team to ensure notification of all referrals is obtained.
    • Performs Intake and Referral functions including: capturing accurate patient information, data entry of referral information, relevant medical history and current orders into EMR including coding of primary home care diagnosis.
    • Obtain initial F2F documents as well as patient discharge paperwork from referral source according to agency and regulatory standards.
    • Verifies patient’s primary insurance, and communicates this information to the Billing Dept to request initial authorizations as needed. Work closely with the Billing Dept to provide any clinical information that may be required to obtain initial authorization.
    • Track insurance authorizations and provide ongoing clinical information required to obtain ongoing authorizations as required and communicate this information to the clinical staff.
    • Notifying Clinical Managers and Scheduling Coordinator of all referrals, including any pertinent clinical issues that may require assignment to clinician with specialty. Responsible to communicate any changes in referral status to Clinical Managers and Scheduling Coordinator.
    • Assists agency management and office staff in the maintenance of the patient medical record including EMR and paper files as appropriate and per agency policy/protocol.
    • Notify Home Care Liaisons of any active patients that have been hospitalized. 
    • Responsible for enrolling all identified patients in Telehealth. This includes all aspects of enrollment- Completion of enrollment information to be sent to Telehealth monitoring company, ensuring telehealth machine is available for the clinician to pick up with all required documentation included and communicating to Clinical Managers and Scheduling Coordinator when Telehealth visit is to be scheduled.
    • Daily monitoring of Telehealth readings and ongoing communication to Clinical Managers and Clinicians regarding clinical concerns. This may also include communication to Scheduling Coordinator if it is determined clinical visit needs to be scheduled.
    • Provides other related duties as required or directed by home health administration.
    • Maintains compliance with policies, procedures, and regulatory matters.
    • Participates in personal and professional growth and development including staff meetings and in-service education.
    • Promotes and maintains an agency environment that is in compliance with federal, state, and local regulatory agencies.
    • Communicates with patients, families, and other health professionals in a manner that conveys respect, caring, and sensitivity.
    • Provides answering service with on call details during closed office hours
    • Responsible to rotate to cover Intake Responsibilities on holidays as required.
    • The above covers the most significant responsibilities of the position. It does not however, exclude other duties which would be in conformity with the level of the position.



    • Must be an RN or LPN
    • Certified Home Care experience – 1-2 years required including working knowledge of Medicare regulations for Certified Home Care. 
    • Experience with identification and coding of primary/secondary home care diagnoses
    • Experience with obtaining insurance authorizations
    • Telehealth experience
    • Proficient computer skills.
    • Experience with data entry in EMR
    • Good verbal and written communication skills.
    • Must be motivated to learn and flexible to change.
    • Strong customer service skills.
    • Prior Experience as Intake/Office Nurse preferred
