    The Rehab Manager will have direct mentorship from a 25 year plus PT and leader in the Home Health Industry. We strive to engage our employees which has resulted in low turnover for this rehab team. We have a geriatric certified physical therapist on the team who was also the Home Care Alliance Clinician of the year in 2018. This position is new and is the result of growth in our agency and will be responsible for managing a team of 20.

     

     

    Responsibilities

    • Must be available to fulfill responsibilities during all business hours 
    • Must provide oversight of patient care services and personnel in conjunction with Clinical Director for Home Health 
    • Oversight responsibilities include making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of individualized plan of care in conjunction with Clinical Director for Home Health 
    • May be pre-designated in writing by the agency Administrator to assume the responsibilities of the Administrator in his/her absence.
    • Plans, develops, organizes implements, evaluates, and directs clinical programs and associated activities.
    • As a member of the Home Health leadership team, collaborates and communicates effectively with quality performance and leadership
    • Collaborates with administration in planning programs and activities
    • Analyzes and evaluates the service delivery needs and trends on a regular basis.
    • Ensures coordination and implementation of established client care plans and related activities
    • Makes recommendations to the Clinical Director concerning the hiring, performance management, promotions, and termination of clinical staff.
    • Ensures performance objectives, including productivity, provides feedback on performance consistently, and conducts performance appraisals as required.
    • Maintains appropriate staffing to assure timeliness and quality of services provided.
    • Ensures that efficient systems, procedures, and controls are established for all clinical operations
    • Provides support to on call clinician for evenings and weekends in conjunction with Clinical Manager for Nursing.
    • Provides clinical supervision to Rehab staff in the delivery of client care and makes joint visits with all professional staff at a minimum of annually. Additional supervisory visits will be required if deemed necessary to ensure competency in the field.
    • Monitors staffing patterns and needs using performance indicators
    • Implements and updates the staff orientation and skills requirement on an ongoing basis.
    • Acts as a resource to Clinicians in regards to navigation and problem solving of EMR system
    • Coordinates and oversees case reviews
    • Supervises the timeliness and quality of documentation of all staff
    • Works closely with the Clinical Director to plan and implement policies and procedures relative to clinical operations.
    • Demonstrates knowledge of OASIS documentation and is proficient in assessing clinicians accuracy in documentation during OASIS Inter-rater reliability visits.
    • Participates in the development and implementation of policies and procedures
    • Reviews complaints and grievances and makes necessary oral/written reports to the Clinical Director
    • .Supports customer service program by promoting and maintaining a responsive, knowledgeable, and accessible approach to all customer contacts.

     

    Qualifications

    • Licensed to practice as a Physical Therapist in the state of Massachusetts. . A minimum of 1-year management experience in a certified home health agency required.
    • Minimum of 2-3 years experience required in a certified home health agency.
    • Evidence of leadership and effective communication abilities.
    • Ability to relate and work well with others within each level of authority and responsibility.
    • Demonstration of knowledge of community health care, governmental licensure regulations and Medicare criteria.
