    The Director of the Clinical Liaison Team is a professionally licensed RN who has graduated from an accredited school and is currently licensed to practice in the state of MA. She/he supervises transitions in care for patients to their place of residence. She/he is responsible for the day to day operations and coordination of patients referred for HCBS services from a health care entity or provider to their place of residence. The Director evaluates program effectiveness in partnership with and under the direction of the Executive Director for HCBS. She/he also maintains the successful achievement of the Agency’s mission, goals, and objectives


    • Participates in developing standards which ensure safe and therapeutically effective service to patients and families. Has joint responsibility with the Administrator for seeing that standards are met.
    • Plans, develops, organizes, implements, evaluates, and directs the clinical liaison program.
    • Meets with administration of the Agency and the system in planning programs and activities.
    • Makes recommendations to the Executive Director concerning the hiring, discipline, promotions, and termination of clinical liaison staff.
    • Establishes performance objectives, provides feedback on performance, and conducts performance appraisals annually and as needed.
    • Maintains appropriate staffing to assure timeliness and quality of services provided.
    • Ensures that efficient systems, procedures, and controls are established for all HCBS liaison operations.
    • Performs components of liaison activity that require expertise; i.e. for example acquisition of new key accounts and HSL internal referral accounts.
    • Monitors staffing patterns and needs using performance indicators.
    • Participates in client care conferences as requested
    • Works closely with the Executive Director to plan and implement policies and procedures relative to clinical liaison operations.
    • Assures effective coordination between clinical staff and business office staff so that paperwork necessary for payroll and billing is submitted on an accurate and timely basis.
    • Participates in the development and implementation of policies and procedures and interprets them as indicated.
    • Reviews complaints and grievances and makes necessary oral/written reports to the Executive Director.
    • Supports customer service program by promoting and maintaining a responsive, knowledgeable, and accessible approach to all customer contacts.
    • Transitions patients from referral source to patient residence by establishing and maintaining relationships with community partners/referral sources and gathering all pertinent information to optimize safe transition.  
    • Keeps referral partners informed of patient status by collaborating with HCBS leadership team to remain informed of unexpected challenges or changes in status of patient.
    • Serves and protects home care agency by adhering to professional standards, home care policies and procedures, federal, state, and local requirements, and professional and licensing standards.
    • Updates job knowledge by participating in educational opportunities.
    • Is able to access reports to track volume by service area, referral sources and physicians.
    • Serves as a resource and support to patients and identifies and responds to safety concerns of patients.
    • Maintains compliance with policies, procedures, and regulatory matters.
    • Promotes and maintains an agency environment that is in compliance with federal, state, and local regulatory agencies.
    • Participates in personal and professional growth and development including staff meetings and in-service education.
    • Enhances HSL HCBS service reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
    • Performs other duties and activities as delegated by the Executive Director.


    • Graduate of an Accredited School of Nursing.


    • Current License with Massachusetts State Board of Nursing.


    • Five years Clinical Liaison experience preferred


    • Good verbal and written communication skills.


    • Must be motivated to learn and flexible to change.


    • Computer literacy required.



    • Must be able to work independently.



    • Current CPR certification.
