Overview

The MDS/Quality Nurse is responsible for the accurate and timely completion of all Minimum Data Sets (MDS) for all patients in assigned units and for coordination in quality efforts to support on-going regulatory compliance.

 

This position is responsible for gathering accurate clinical information on all patients in assigned units in order to accurately complete the MDS.  The nurse should seek to incorporate all members of the team into the MDS completion process via staff education and real time communication. All MDS should be completed in order to guide the delivery of health care and quality of life services for each patient. 

 

In addition to accurate, timely and complete MDSs, the MDS/Quality Nurse is responsible for on-going quality compliance on the assigned units.  This position will conduct document and medical record audits to assist the nursing and clinical teams with maintaining regulatory readiness and achieving target quality indicators. Maintaining an on-going Quality Assurance, Process Improvement program is critical in maintaining excellence. This position will contribute to monitoring quality indicators such as falls, pressure ulcers, customer satisfaction, mobility, etc. The nurse will identify areas for opportunity in documentation and regulatory compliance and provide feedback and education to clinical teams on their findings.

Responsibilities

  • Accurate and timely completion of the Minimum Data Sets for all patients on assigned units
  • Inclusion of all team members in understanding and using the MDS as a key tool in the provision of daily care
  • Audit of MDS completed by nursing team staff supports
  • Assistance in the coordination and education of staff nurse team members on MDS Assessment completion
  • On-going documentation reviews to optimize the accuracy of the documentation and the clinical outcomes measured by the MDS such as the Quality Measures
  • Effective communication with all team members on any issues related to the accuracy and/or timeliness of the MDS and other relevant documentation that impacts regulatory readiness and clinical excellence
  • Monitoring of key quality indicators such as falls, pressure ulcers, infections, etc.
  • Support for maintaining constant survey readiness
  • Participation in committee and process improvement work as it pertains to compliance with regulatory standards and quality compliance
  • Facilitate special projects as project leader as required
  • Timely reporting of any documentation, clinical or compliance concerns to the Nurse Manager, Quality team, Professional Development and other clinical teams as necessary

Preferred Qualifications

  • Current MA Registered Nurse License
  • MDS Training
  • Computer literacy
  • Excellent verbal and written communication skills
