Overview

The Wellness Coordinator is a key staff person, responsible for all aspects of resident services at the Leyland Community (43 apartments located in Upham’s Corner in Dorchester). The position operates with a significant amount of autonomy as the only full-time resident services staff person on site. HSL provides enhanced housing with services to seniors living in affordable housing locations, and at partner housing locations, with the goal of improving quality of life and supporting independent living. At its core, enhanced housing with services is a proactive approach wherein resident services staff regularly and actively reach out to each individual resident to engage with them around their health and wellness, identify areas of need/risk and provide intensive, individualized support as needed and desired by the resident.

 

The Wellness Coordinator engages residents in wellness assessments and health education programs, connects residents to needed services, provides a wide variety of case management tasks, ensures that all interventions are documented and tracked and works to develop and strengthen partnerships with community provider organizations. In addition, they ensure that residents are connected to programs and services that are educational, recreational, cultural, social, and/or multigenerational in nature. The Wellness Coordinator works in close partnership with the other full-time staff person on site – the Maintenance Technician – to ensure that the community provides the highest level of housing with supportive services to all of the seniors who live there.

Responsibilities

• Partners with the housing teams to provide intensive case management services to residents.
• Conduct wellness assessments of residents to determine needs and goals; develop wellness plans addressing physical, social, cognitive, spiritual and mental health domains.
• Plans and coordinates programs that are educational, recreational, cultural, social, and or mutigenerational in nature.
• Coordinate with primary care physicians, mental health providers and hospitals.
• Coordinate with residents to complete files including important health care information, end of life planning, and emergency contacts.
• Communicate with all providers to relay important information (changes in condition, transitions between settings, changes in behavior/activity).
• Promote self-care among residents through individualized coaching to identify personal goals and implement programs and services that support those goals as well as coordinate with care providers.
• Support on site services and connection to required off site services by following up with at risk residents to ensure adherence to health and wellness related activities.
• Partner with and make referrals to all appropriate local service providers, for example: Visiting Nurses Associations, ASAP’s, Rehabilitation Services, PACE Programs, Adult Day Health Programs, Memory Evaluation Programs, etc.
• Participate and/or lead care planning meetings at the housing site.
• Assess the needs of residents with dementia and make appropriate referrals as needed.
• Work with the Wellness Nurse to plan, schedule and implement Evidenced Based Programs.
• Collaborate with housing staff to ensure that residents are connected with all assistance programs for which they are eligible.
• Work with local hospitals, HSL Home Care and/or VNA case management to ensure safe discharges with appropriate service coordination.
• Assist residents with transition to other levels of care as needed; conduct family meetings and attend off-site team meetings when necessary to coordinate care and discharge planning needs.
Assist with operation and administrative issues for residents. For example, calling vendors if something breaks down, assisting with faxing or copying something for residents.”
• Offer advocacy for residents and serve as a liaison for families.
• Maintains work documentation in required databases.
• Performs other related duties as required or directed.

Required Qualifications

  • Bachelors degree required with at least 2 years of experience working with seniors, preferably in a case management role 
  • Must have compassion for and a desire to work with the senior population 
  • Ability to innovate, think strategically and conceptually, manage multiple projects simultaneously and problem solve and handle difficult situations 
  • Must be a professional, proactive, collaborative, conscientious, organized and results-oriented individual. Must have an optimistic and positive demeanor, excellent oral and written communication skills, good intuition and sound judgement. Must be motivated to learn and flexible to change.
  • Experience working with individuals from diverse multicultural backgrounds and communities required 
  • Exceptional interpersonal skills with the ability to work independently, be a strong collaborator and team player, and exhibit multicultural understanding when working with clients and partners 
  • Computer literacy; experience with Windows, Word, PowerPoint, and Excel 

Preferred Qualifications

  • Masters degree in a human service field and 1-2 years of experience working as a case manager or care coordinator in aging services preferred
  • Verbal and written fluency in Cape Verdean Creole, Hatian Creole, Spanish or Vietnamese is highly desired 
  • Certification in Dementia Care preferred 
  • SHINE Counselor preferred
