The Skilled Nursing Facility (SNF) at Orchard Cove is a 45 bed unit within a continuing care retirement community that provides long term care and short term rehab services. The unit is committed to person-directed care. We are developing a unique and innovative program to help each resident achieve what matters most to them. The Social Worker provides emotional support to residents and their families during key life transitions involving the SNF such as admission; adjustment to the unit; discharge from the unit; decline in level of care and the dying process. The Social Worker shall advocate for the resident and his/her family on an individual basis in an effort to best support the resident’s autonomy and dignity. The Social Worker will serve as a key communicator between residents, families and staff so each group gets to know one another and understands each other. The Social Worker will assist each resident in maintaining his/her highest practical level of physical, mental and psycho-social well being.

The Social Worker shall be responsible case management services for short-stay residents, providing Medicare Notices of Non Coverage to prepare for status changes; and assisting patient/residents in securing appropriate home care services to ensure a proper discharge plan.

  • Empathy for others and a core understanding that our patients/residents and their families are experiencing significant life transitions
  • Mediator skills to deal with resident, family and staff dynamics
  • Clinical knowledge and an understanding of proper usage of a medical record
  • Able to function as an integral member of the clinical team

 

Position Responsibilities: 

  • Provides emotional support to residents and families during times of transition; adjustment to life in a nursing home; acceptance of change in medical condition and end of life
  • Completes all admission and discharge paper work with resident/family
  • Serves as an advocate for resident/family
  • Key communicator between resident/family/staff
  • Coordinates admissions and discharges
  • Delivers all necessary notices including the Medicare Notice of Non Coverage; the Advanced Beneficiary Notice; etc.
  • Completes discharge planning and ensures a safe discharge for all residents leaving the unit

Qualifications

  • Bachelor’s degree in Clinical Social Work (Masters Preferred)
  • Licensed Clinical Social Worker
  • Excellent communication skills
