Responsibilities

The Clinical Nurse Specialist is responsible for the development of educational programs for Clinical Teams within Home Health and Hospice.

The Educator creates an annual plan for all aspects of professional development for clinical teams and partners with colleagues and key stakeholders across HSL to optimize the continuum of services and care.

This role is guided by the quadruple AIM - optimal care and experience for the senior and their family, applying a population health focus in a cost-conscious way, and recognizing employee engagement as a key driver toward excellence.

 

 

II. Position Responsibilities:

 

  • Designs, implements, and evaluates
    the Home Health and Hospice clinical orientation program that is customer focused, competency based, and in compliance with regulatory standards and agency policy.
  • Accompanies clinical staff during home visits to identify knowledge gaps. Observes skill level and accuracy with adherence to regulatory requirements and best practice clinical skills, reinforces regulatory guidelines, OASIS accuracy, agency policies and procedures and facilitates optimum levels of clinical practice.
  • Develops and provides educational programs that will improve individual and group performance in case management, clinical care, PDGM, and the Quadruple AIM.
  • Provides individualized education sessions based on review of findings from home visits and in collaboration with Quality and Patient Safety team members and Clinical Leadership, and as identified during and after completion of orientation.
  • Develop the annual education plan, annual competency program, new program protocols, and attaining relevant goals in collaboration with other leaders.
  • Provides excellence in service to internal customer base of employees and external customers including patients, referral sources and physicians and in accordance with agency standards.
  • Works collaboratively with all team members, executive, and clinical leadership to coordinate high quality educational activities based on identified need.
  • Participates in staff meetings to facilitate ongoing education as it relates to clinical, quality, regulatory and compliance standards.
  • Follows agency policies and procedures and adheres to standards of practice within agency guidelines.
  • Administers care within scope of practice, as indicated.
  • Supports in the implementation and evaluation of the agency quality program to assure quality outcomes are achieved.
  • Audits clinical records in preparation for home visits and participates in initial and follow up home visits in order to determine learning needs and to assess for trends.
  • Utilizing Strategic Healthcare Programs (SHP), data analytics software system, reviews outcome data of both agency and individual clinicians to identify trends, areas requiring outcome improvement, and establishes, implements and revises as necessary a response plan
  • Participates on the Home Care Quality and Safety Committee and other committees as requested.

Collaborates with other members of the professional development team to develop and implement education initiatives.

Required Qualifications

  • Licensed Registered Nurse in the state of Massachusetts.
  • Baccalaureate degree from accredited program, Master’s degree in Nursing Education
  • Must have recent home health experience for minimum of two years with demonstrated experience in OASIS, Medicare Home Health Regulations, interpretation of outcome data, and clinical staff education.
  • Possess strong leadership skills, effective communication skills, ability to independently work in a team environment.
  • Proficient in Microsoft Word, PowerPoint, and Excel
