We are focused on providing care and support that will help seniors recover after an illness, manage chronic conditions, adjust to a new limitation, and support your goals for health and wellness.

Responsibilities

  • Make Home Visits to patients in designated geographic territories
  • Evaluate clients, develop appropriate treatment goals and methods, implement the physical therapy treatment plan, and complete all related documentation and record keeping regarding these services. 
  • Services include application of therapeutic techniques in areas such as gait/ambulation, muscle strength, coordination, endurance, mobility, posture control, sensory awareness, sitting and standing skills, balance and orientation. 
  • Select physical agent modalities suited to the individual’s needs, physical capacity, activities and diagnosis. 
  • Instruct client in the skills and techniques required for participation in mobility activities, and evaluate client progress, attitudes and behavior as related to rehabilitative potential. 
  • Contribute to client care, staffing conferences and other related meetings. 
  • Supervise supportive personnel in accordance with state and professional standards. 
  • Clean, maintain, identify, and report the need for repair of equipment as necessary. 
  • Ensure that residents and families receive the highest quality of service in a caring and compassionate atmosphere, which recognizes the individual’s needs and rights. 

Qualifications

  • Current licensure as a Physical Therapist in Massachusetts 
  • Requires 2 years of previous experience in home care, outpatient services for elderly, or skilled nursing facility
  • Must be a licensed driver with an automobile that is insured in accordance with state and/ or organization requirements and is in good working order, or other reliable means of transportation.
